Please submit each of the linked forms above. Fill out the account paperwork and send back all pages to the address above open an account. We will call you with receipt of paperwork and your account number. Due to recent changes in securities laws and the Patriot Act, please send a copy of your Drivers Liscence as well. All documentation sent to R.F. Laffety is kept in strict confidence and is not shared with any third parties.

Account Transfer Form

Please download and complete this form to transfer an account to R.F. Lafferty & Co. You can either transfer an entire portfolio or specific positions. Either way, please include a copy of your most recent statement.

 

Traditional IRA Account Application
Roth IRA Account Application

Fill out this form in order to open an IRA account. Please also fill out the new account application.

Corporate Resolution

Fill out this form in addition to the new account application to open a corporate account.

Since corporate accounts are registered in the name of the business, the resolution specifically defines who has authority to buy and sell securities. This form also must be notarized.

Limited Trading Authorization

Fill out this form to give another individual not registered on the account the ability to buy or sell securities for the account.

Incoming Wire Instructions

Fill out this form to wire money into your R.F. Lafferty account from another institution.

Stock Power

A stock power is required when an individual or one of the members of a joint party has not signed the back of the original certificate. A completed stock power allows the certificate to be deposited into the individual's account, so we may then release it to our transfer agent.

Trust Certificate

This form certifies who the trustee on the account is and defines what powers that person has. Only the trustee is authorized to trade securities or administer in a trust account. This form must be notarized.

W-8 Form

This Internal Revenue Service form  "Certificate of Foreign Status" is required of all non-U.S. Citizens and non-U.S. residents who hold accounts with us. It is used for tax purposes.
